primary care networks
We can support your Division to explore service plan development, create an implementation plan, and ultimately deliver on those plans. Changing the primary care model in a region can be challenging and time-consuming. Patient medical homes
Patient Medical Homes (PMH’s) are the foundation of the primary health care system. With challenges across the province, Hive can support Divisions who have prioritized PMH work, and deliver both clinic-specific and member-wide resources to increase efficiency, promote member engagement and decrease physician burnout promote a more sustainable work-life balance.
HOW WE CAN HELP
PATIENT MEDICAL HOMES
Recommend programming and develop implementation plans based on membership needs to support sustainability of clinics. Some examples include designing membership-wide recruitment and retention strategies, creating patient-attachment focused initiatives and nuanced clinic optimization programs.
Work with specific clinics to explore solutions for pain-points (such as staff performance, task delegation, etc.) and help ready the clinics to better engage with upcoming initiatives.
Design surveys, prepare engagement guides and facilitate discussions to support the collection of relevant information from members and other stakeholders.
Create decision tools and operational resources for Division’s membership with the goal of increasing the sustainability of clinics in the region.
Delivering educational sessions and series to teach members and office staff about important practice management topics such as finance, HR and operations.
PRIMARY CARE NETWORKS
Provide support for various stages of planning including development of service plans and creation or refinement of implementation plans.
Support the roll out and placement of PCN resources both in-clinics or in-hub locations in the region
Help improve member engagement strategies to further the Division’s change management efforts in the region.
Advise PCN Managers and Directors on service and implementation plans to create sustainable and manageable changes in the system.
Work with PCN team and evaluators to ensure proper benchmarking and capturing of important metrics to measure success of PCN resources and initiatives.
Unpack important operational processes and procedures to help align solutions across stakeholders in the PCN (eg. Forms, referral processes, patient pathways, etc).
Support clinics in alleviating clinic-specific hurdles to improve uptake and utilization efficiency of PCN resources.
Past Division Projects

The Vancouver Division responded to member requests for practical tools to help clinics and managers analyze or modify different operational elements of their practices to make informed decisions. Hive worked closely with the Division to design and create a variety of tools catered to support Division members with their most commonly surfaced questions and concerns. For example, Hive developed a tool based on a correlational analysis of respective wages and common responsibilities for MOAs in junior, mid-level, senior and managerial roles across the city. The tool was meant to guide clinic decisions related to incentivization of staff, development of competitive wage ranges, and delegation of tasks to new and existing staff.

The South Island Division of Family Practice identified, through in-depth member engagement sessions, the demand for external business advisory support to help organize and more efficiently run their practices. The SIDFP partnered with Hive to support twenty clinics in the region with projects related to the clinic-identified issues, challenges, and concerns. Through each clinic project, Hive worked with physicians and clinic leads to address their challenges and create resources, tools, and solutions to navigate the issues. The Division concurrently hired an in practice consultant, project manager and used the services of two PCN change managers to continue the work initiated by Hive to establish a sustainable and effective support system in the South Island region.
